Fire Instructor Renewal Form

In order to renew your Fire Service Instructor status with the Division annually, you must complete the required instructional activity and continuing education requirements.  For those who are “staff” instructors, the Division tracks your teaching activity and there is no need to complete the “INSTRUCTIONAL ACTIVITY portion of the form.  Simply write “ON HORIZON” in that section and we will verify your activity.  You WILL be required to document your continuing education on the renewal application.  ALL STATE instructors MUST complete the INSTRUCTIONAL and CONTINUING EDUCATION fields on this application as the Division does not track your teaching activities.

  • Please use the fields below to enter your instructional and continuing education hours for this renewal season. Before doing so, please read the following guidelines to assist you in proper completion of the form. You must document no less than 16 hours of instructional activity and 8 hours of continuing education hours. STATE INSTRUCTORS - must fill out both instructional and continuing education activity fields. The Division will verify your information once submitted. STAFF INSTRUCTORS - If you were assigned classes through the HORIZON STAFF SCHEDULER SYSTEM, simply indicate "ON HORIZON" in the instructional activity area as your instructional activity is all ready on file with the Division. You must still complete the continuing education area. SPECIALTY INSTRUCTORS - are not required to complete a specified amount of instructional activity or continuing education. You may elect to provide information to the Division in either of these fields.
